How to submit a Certificate of Substantial Performance

Complete Form 6 with all areas filled in. Make sure it is signed by a payment certifier!

Get Form 6: If you don't have a copy of Form 6, download it in Microsoft Word or PDF format (you will need Adobe Reader to open the PDF form). Right-click on the link and select “Save Target As” to save a copy of the form. Open the Word or PDF document, fill in the necessary fields, print, sign and fax back to Daily Commercial News for publication.

Fax your form to 1-800-947-7966. Make certain that you send a cover sheet in case we need to call you! If we receive your ad by 3:00 p.m. Monday through Friday, your ad will be published three business days later. Please call us at 905-752-5482 if you need any assistance.

We will invoice you for your ad and you will also receive a copy of the ad in a certificate. The invoice and copy of your ad will arrive approximately one week after the ad runs.
